Dear Patient, 

 

We would like to welcome you to Comprehensive Physical Therapy and to thank you for selecting us to provide your physical therapy care. Our experienced and dedicated staff is committed to quality service and individualized attention. 

 

Your initial evaluation will last approximately 1 hour. It will focus on the completion of a comprehensive evaluation and assessment of your condition. The data gathered will enable your therapist to provide more individualized treatment and to communicate more effectively with your physician. At the completion of the evaluation if needed a consultation letter will be sent to your physician outlining our findings and plan of care. 

 

Each additional visit will last anywhere from 45 minutes to 11⁄2 hours depending 

on your individual diagnosis and rehab program. We will provide you with a standard gown, however if you would feel more comfortable in your own gym clothes, please feel free to wear them. 

 

Prior to your initial visit, a member of our business office will gather your insurance information and verify your individual coverage. A thorough explanation of your benefits will be given to you before physical therapy is 

initiated. 

 

We have designed our office hours in hopes that they are most convenient for you. Our extended hours are on Monday, Wednesday and Friday from 8:00 a.m.- 7:00 p.m. and on Tuesday and Thursday and Saturday from 8:00 a.m. to 12:30 p.m. In order to insure times that fit into your schedule we recommend that you keep your appointments scheduled two weeks in advance. Kindly provide 24 hours advance notice for any cancellations.
